To register with our practice, a registration form, which is available at reception, must be completed. Forms are also available to download from the above "Registering Online" tab.

All newly registered patients will be asked to attend the practice nurse for a health check.  This will last for about 20 minutes and allows the nurse to take note of any past medical history while we are waiting to receive you medical record. Please ensure this appointment is kept.

You must bring 2 forms of identification with you to a new patient medical.  One of which MUST be photographic.

The practice does not discriminate on the grounds of race, gender, class, age, religion, sexual orientation or appearance, disabling or medical condition. It is necessary for the practice to record ethnicity for all our patients in agreement with our contract with  Greater Glasgow and Clyde Health Board.
